32 | Based on interviews conducted and records reviewed, it was determined that on 02/25/2025, and again on 02/27/2025, in classroom Toddler A, there were two (2) staff supervising nine (9) children for a duration of up to three (3) minutes. Five (5) out of eight (8) staff interviewed admitted that the facility has operated classrooms out of teacher-child ratio on multiple occasions and expressed concerns with staffing at the facility. Director stated that there have been occasions when parents have been asked to wait with their child upon drop-off until sufficient staff arrive to avoid being out of ratio.
Based on LPA interviews conducted, and records reviewed, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. One (1) type B deficiency of California Code of Regulations, [Title 22, Division 12, Chapter 1, Section 101416.5 (b)], is being cited on the attached LIC 9099D.
