25 | On 7/23/2024 at 3:01 pm an office meeting was conducted with Director Ana King to discuss recent facility history. Present at the meeting was Licensee Representative Tara Martinez, Licensing Program Manager (LPM) Cynthia Gray and Licensing Program Analysts (LPAs) Adrian Castellon and Julieta Abrego.
On 5/24/24, the facility was issued an Infant Care Activities Type A citation. On 9/27/23, the facility was issued a Food Services Type B citation. On 9/18/23, the facility was issued a Type B Personal Rights citation. On 8/24/23, the facility was issued a Capacity Determination Type A citation. On 6/17/22, the facility was issued a Type A Care and Supervision citation. On 11/8/21, the facility was issued a Type B Infant Care Personal Services citation. The facility was previously placed on a Non Compliance Plan from 12/7/17 through 12/7/19.
The facility has received three (3) Type A citations and three (3) Type B citations between complaint allegations and incidents since the beginning of 2021, including the following citations: personal rights, responsibility for providing care and supervision, food services, capacity, infant care personal services, and infant care activities.
LPM and LPAs discussed the above citations with Licensee Representative and Director. Licensee Representative and Director were provided the following regulations: Sections: 101214 Accountability, 101415 Infant Care Center Qualifications and Duties, 101429 Responsibility for Providing Care and Supervision, 101223 Personal Rights, 101179 Capacity Determination, 101430 Infant Care Activities, and 101227 Food Services.
Licensee Representative and Director were informed that repeated violations or failure to comply with licensing laws and regulations may result in administrative action taken by the Department. Licensee Representative and Director stated they understood and will ensure the facility complies with all regulations and laws governing Child Care Centers.
