On 5/24/24 at 10:30am, Licensing Program Analyst (LPA) Adrian Castellon conducted an unannounced inspection to deliver complaint findings for the allegations listed above. LPA met with Assistant Director Rosa De La Torre to discuss the purpose of the inspection and toured the facility. It was alleged that Staff do not follow safe sleep practices for infants under twelve months old. Based on the interviews conducted, sleep logs obtained, and staff admission, the allegation is substantiated. As such, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Violations of the California Code of Regulations, Tittle 22, are being cited on the attached LIC9099D. A Type A citation is issued. Exit interview was conducted, report reviewed, and Appeal Rights discussed with licensee. LPA informed licensee to provide a copy of this report that documents Type A citation to parents/guardians of children currently enrolled by the next business day or the next day children are in care, and to newly enrolled parents/guardians for 12 months from this date. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file. A Notice of Site Visit was given and must remain posted on, or immediately next to, interior side of the main door for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Facility management will implement Child Supervision Record reviews on a two hour basis for one month period. Beginning on 5/28 thru 6/28/24. Records will be submitted to Licensing Office. SIDS training will be completed by all infant license staff through Kindercare Website (Experience Everything).

12 month of age on their backs for sleeping. This requirement was not met as evidenced by infant in care was allowed to sleep on their stomach for approximately three hours. This was documented on Kindercare app sleep log. This is an immediate threat to the health and safety of children in care.
