On 04/30/2024 at 12:35pm, Licensing Program Analyst (LPA), Selina Siao conducted an unannounced Case Management inspection for adding a toddler option and a decrease in capacity. Upon arrival, LPA met with Director Elizabeth Ferrusca and conducted a tour of the facility to gather census. Present during today’s inspection were 36 children napping on cots and mats supervised by 6 staff members in classrooms 1A, 2 and 3. Appropriate ratios and supervision were observed. All staff members have the required background clearances and are associated to the facility. All indoor and outdoor activity space utilized for the children were inspected today. LPA informed Director that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. Facility is currently licensed to care for 60 preschooler ages 2 to 5 year old in classrooms 1, 2 and 3. Facility has requested to be licensed for 56 children including 16 toddlers (ages 18 to 36 months) in classrooms 1 and 2 (facility added a wall in room 1 to divide the room.) and 40 preschoolers ages 3 to 5 year old in classrooms 3 and 4. Hours of operation is Monday to Friday 7:30am to 6pm.

LPA continued to tour the facility and remeasured room 2 and check the outdoor activity space. Total indoor activity space measured to be 847.19 square feet for the toddler classrooms 1 & 2 and 1618.88 square feet for the preschool classrooms 3 & 4, which is sufficient to accommodate the requested capacity of 16 toddlers and 40 preschoolers. LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms and outside via portable Igloo with plastic cups for the children. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 4/24/2024.

LPA observed a total of 4 sinks and 4 toilets available for preschool children and 3 sinks and 1 toilet for toddlers. Facility will purchase a potty chair for the toddlers. These are sufficient to accommodate the requested capacity of 56 children.

The facility currently has a fully fenced playground area. The chain link fencing is at least four feet high. The preschool playground was previously measured to be 2887 square feet, which is only sufficient for 38 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. The toddler playground was previously measured to be 1765 square feet which is sufficient for the requested toddler capacity of 16 children. Shade is provided via large shade tarp structures at both playgrounds. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure on the preschool playground that appears to be age appropriate. There is adequate cushioning in fall zones of climber provided by rubber cushioning. LPA observed all hazardous items on the playground to be inaccessible to children. Director was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

The following corrections are needed prior to the issuance of adding the toddler option and the change in capacity:



· Have one potty chair available for the toddlers.
· Submit an outdoor playground waiver for the preschool children to share the playground in classrooms 3 and 4. The waiver has to be approved by the department prior to changes to the license.

Director understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.
