On 03/10/25 at 2:40pm Licensing Program Analysts (LPA), Samantha Clenista and Selina Siao conducted an unannounced case management inspection to address facility’s request of increasing their overall capacity from 70 to 105 children. Upon arrival, LPAs met with Center Director, Sarann Sok. LPAs took a tour of the licensed areas and observed a total of 59 children with a total of 5 staff members. Facility is requesting to be licensed for 105 school-aged children (ages TK to 5th grade) in the following rooms: library, multi-use room and B37. Facility is already licensed in the library and multi-use room, and is requesting to add B37 to their license. Fire clearance was granted on 02/27/25. Facility is exempt from indoor and outdoor square footage per Title 22, sections 101538.2(c) and 101538.3(c). No measuring of any sort was conducted today.

LPAs inspected all indoor and outdoor play space. The children uses the elementary's outdoor playground area located outside of room B37 which consists of blacktop areas that include basketball hoops, wall ball and four square. Restrooms designated for children were observed to be separate and allowed for individual privacy (labeled on sketch). Ms. Sok was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

There is a separate staff restroom located in the library building. There is an operational carbon monoxide detector located in the multi-use room and Room B37. First Aid Kit is kept in the Multi-Use Room. When medications are on site (depending on the medication), Ms. Sok stated that they are in a locked cabinet in the Multi-Use room. Cleaning supplies and other toxics are inaccessible to children through latches, locks and/or placed on high surfaces. Drinking water is readily accessible via filtered water fountains. Ms. Sok is reminded to keep anything that reads, “Keep Out of Reach of Children” inaccessible to children.



A notice of site visit was given to Ms. Sok and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview was conducted and report was reviewed with facility representative. Prior to LPAs granting facility the increase of capacity, facility shall fix two ceiling panels located in B37 and submit an updated facility sketch detailing children's outdoor play space and appropriate room names, staff and children restrooms.
