Licensing Program Analysts (LPAs) Jennifer Lott and Nancy Diaz made an unannounced visit to conduct an annual inspection. LPAs were greeted by Director, Sandra George and granted entry after identifying themselves and disclosing the purpose of their visit. During today's inspection, there were 24 children with 6 staff present. Appropriate ratios and capacity were observed.

Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Drinking water is readily accessible. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area is clean and free of hazards. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste did not have tight-fitting covers. Storage areas for poisons are locked. Director stated there are no firearms or other weapons on the premises. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility.

Outdoor play area is a fenced playground with sufficient material for cushioning. There are no bodies of water at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has drinking water readily accessible and playground did have an area with a potential tripping hazard.

6:6 Personnel records were incomplete, however, at least one staff member has current CPR and First Aid certifications. 5:6 staff have not completed mandated reporter training and required immunizations. Sign ins were reviewed and were missing full signatures. 9:10 children's records were incomplete. A review of staff records on this date indicates 1 staff did not have a criminal background clearance / transfer. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility does not care for infants, however LPA discussed the safe sleep regulations with the director, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director will update the following forms and send to CCL; emergency disaster (LIC 610), personnel report (LIC 500), emergency designation form (LIC 308) and Administrative Organization (LIC 309).

Deficiencies are being cited per Title 22, Division 12 and noted on the attached 809D. Civil penalties are also being issued on the attached LIC 421BG for no background clearance in the amount of $500.00.

LPAs Lott and Diaz informed Director, George that this report dated 11/16/2022 document(s) one (1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health & safety of children in care.
Also, LPAs Lott and Diaz informed Director, George, to provide a copy of this licensing report dated 11/16/2022 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview conducted and report was reviewed with Director, Sandra George. A notice of site visit was given and must remain posted for 30 days.
