On 2/27/2023 @ 9:05AM, Licensing Program Analyst (LPA) Nancy Diaz conducted an unannounced case management inspection. LPA met with Sandra George, owner/Director.

The purpose of this inspection was to serve an immediate exclusion from facility to staff Krysta A. Wolfe.

No deficiency cited today.

Exit interview was conducted with Sandra George. A copy of this report and appeal rights were also provided today. Notice of site visit was provided and observed posted.
