On 1/24/2025 at 8:45 a.m. Licensing Program Analyst (LPA), Adrian Castellon conducted an unannounced complaint inspection for the above allegations. LPAs met with Assistant Director Alexa Lorenzi and advised licensee of the purpose of the inspection and conducted a tour of the facility. There were XX children present during the inspection.

During the course of the investigation, interviews were conducted with reporting party, director, facility staff, and day-care parents. LPA Castellon inspected classrooms.

Based on interviews conducted it was determined staff did not provide adequate supervision to an infant in care and that staff did not ensure facility is kept clean (floor in infant classroom). Infant was eating on a high chair without proper supervision. Although parent was in the classroom and intervened, proper supervision was not provided by facility staff. Floor is not cleaned enough or thoroughly.

Based on interviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations, (Title 22, division 12 & Chapter 3) one (1) Type A citation is being cited on the attached LIC 9099-D. One (1) Type B citation is also being cited on the attached LIC 9099-D.


LPA informed Assistant Director that this report dated 1/24/2025 document(s) (1) Type A citation which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA informed Assistant Director to provide a copy of this licensing report dated 1/24/2025 that documents Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

On 1/24/2025 at 8:45 a.m. Licensing Program Analyst (LPA), Adrian Castellon conducted an unannounced complaint inspection to deliver the findings for the above allegations. LPA met with facility assistant director Alexa Lorenzi and advised of the purpose of the inspection and conducted a tour of the facility.

During the course of the investigation, LPA conducted interviews with reporting party, facility staff, and day-care parents. The facility roster was obtained and reviewed by LPA. Due to conflicting information obtained throughout the course of the investigation and no other witnesses to the alleged incidents, LPA was unable to determine whether or not the allegations occurred. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegations are UNSUBSTANTIATED.
Exit interview was conducted and the report was reviewed with assistant. A Notice of Site Visit was given and must remain posted for 30 days.

Staff will hold a infant staff meeting where Section 101229(a)(1) will be discussed. Staff will submit meeting attendance sheet. Program Specilast will re-train staff on proper supervision. Specialist training will be documented and submitted.

101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. This requirement was not met as evidenced by child not visually supervised at all times while eating and sitting in a high chair. This is an immediate threat to the health and safety of children in care.

Infant classrooms will be cleaned by a professional service by 2/7/25. Staff will be instructed on proper cleaning techniques including cleaning solutions and that plan will be submitted to the SDCCRO along with cleaning order by POC date.
