25 | On 10/21/2024 at 1:40 pm, Licensing Program Analysts (LPAs) Michelle Hood and Shannan Williams made an unannounced case management inspection. The purpose of the inspection is to issue additional citations discovered while investigating a complaint received on 09/10/2024. There were two children with the licensee and one helper present.
On 09/19/2024, during a complaint inspection, LPAs reviewed the facility roster which was not completed or current with the daycare children enrolled at the facility. During an interview, it was disclosed the licensee allowed a daycare infant to use a baby walker while in care. The LPAs reviewed pictures and video of the infant in the baby walker while in the family child care home. The Licensee did not have personnel files for at least two staff. The licensee admitted to not maintaining the personnel records which includes: mandated reporter certification and First aid/CPR training for two staff that transported children to and from school. The Licensee did not obtain criminal record clearance or exemption for two staff prior to allowing them to work in the facility. Based on interviews and review of a photograph, it was determined the licensee violated the safe sleep regulation by allowing a blanket in the crib or play yard while an infant is napping. The licensee did not maintain a completed Individual Infant Sleeping Plan (LIC 9227) for at least one infant in care. Based on interviews, Licensee admittance, and LPAs observation, the LIcensee allowed children and at least one infant to nap on the couch.
LPAs Hood and Williams informed licensee Ana Laura Ruiz that this report dated 10/21/2024 documents three (3) Type A citations which shall be posted for 30 consecutive days as there is/are immediate risks to the health, safety, or personal rights of children in care.
