25 | On 11/13/2025, Licensing Program Analyst (LPA) Hanna Lucas, initiated a case management visit - deficiencies. LPA met with the Licensee, Malicah Walsh. LPA observed two children in care, zero children were under the age of two years old.
During a substantiated complaint investigation, it was determined that Licensee's adult son, Kai Walsh, has personal possessions and maintains a room at Licensee's residence. He has been observed by various parties coming and going from the home several times during the course of the investigation and observed driving Licensee's car.
The Department Investigative Branch (IB) confirmed with the Licensee, Malicah Walsh, that her son, Kai Walsh, who is an uncleared adult, has a bedroom within her home.
During the investigation, interviews were conducted with law enforcement, Licensee Malicah Walsh, Licensee's adult son, Kai Walsh, and additional pertinent parties/witnesses.
Relevant documentation was reviewed, and a comprehensive inspection of the home was conducted. Based on the information obtained during the investigation, it was determined that Kai Walsh resides within the day care home.
TYPE A deficiency is cited on the attached LIC809D. A civil penalty of $500 was assessed and documented on the LIC412BG.LPA informed Licensee that this report dated, 11/13/2025, documents (1) Type A citation, which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
