THIS IS AN AMENDED REPORT DELIVERED ON 6/17/26:
On 5/14/26 at 9:40 AM, LPA Mahjoba Mohsini conducted an unannounced visit regarding the 2/23/26 complaint and was granted entry by Licensee Jaleh Bono; the helper Maria Bono, Licensee's husband Guiseppe Bono, and nine day care children were present. The facility was within ratio and capacity.
It was alleged that the child’s personal rights were violated, and the Department fully investigated through interviews, records, and observations and determined there was not enough evidence to substantiate the allegation involving C1, after which LPA reviewed concerns about diaper area redness. Subsequent interviews and record reviews showed routine care practices with no evidence linking the rash to the facility, and no information was obtained showing that the facility’s actions or omissions resulted in a Personal Rights violation; therefore, there is not a preponderance of evidence to support the claim, and the allegation is UNSUBSTANTIATED.
An exit interview was conducted, the report was reviewed with the Licensee Jaleh Bono, and a Notice of Site Visit was issued and must remain posted for 30 days.
