25 | On 04/14/26 at 9:45 am Licensing Program Analyst (LPAs) Gerald Poindexter and Paul Cortez conducted an unannounced annual inspection. Upon arrival, LPAs met with licensee Deedee Smith. Also, in the home were the licensee’s husband, Brad Smith, and assistants Cameron Smith (licensee’s adult daughter and resident), Kiera McGuire, and Kristina Johnson. Present in the home were 11 day care children of whom one was under 24 months. Licensee states hours of operation is 8 am to 5:00 pm, Monday through Friday. The licensee was provided with the Inspection Checklist (LIC 126). The 2-story home was toured and inspected to ensure an environment safe for the care and supervision of children. The second floor of the home is separated by gates from the childcare area and requires entry from outside.
The licensee has provided adequate space for the children to eat, sleep and play within the home, exclusively on the first floor. Areas used for child care include play room, bathroom #1, living room, dining room, and enclosed backyard. Off limits areas include kitchen, library, bedroom #1, bedroom #2, bathroom #2, laundry room and the entire upstairs and are inaccessible through door clamp lock, door locks, or walled off from the remainder of the home. Playroom, bathroom #1 and backyard are accessed through separate entry/exit. The living is used to care for/separate sick children. The facility has sufficient toys and equipment available. The home has a fenced backyard available for outdoor activities. Licensee understands that visual supervision is required at all times during outdoor activities. No body of water was observed during time of inspection.
There is a fully charged fire extinguisher, smoke and carbon monoxide detector that meet requirements and are operational. There is an outdoor fireplace that is inaccessible to children. Poisons, cleaning compounds, medications and other hazardous items are inaccessible to children via high placement and/or storage in CONTINUED ON PAGE 2
