25 | On 12/15/23 at 9:00am Licensing Program Analyst (LPA) Patrick Ma conducted an unannounced annual inspection, including inspecting new outdoor playstructure for approval. Upon arrival, LPA identified himself and provided his badge to Licensees Tamie Johnson-Schirlls and Amber Schirlls. The one story home was toured and inspected to ensure an environment safe for the care and supervision of children. Present in the home were 2 day care children, including 1 infant.
Licensee has provided adequate space for the children to eat, sleep and play within the home. Areas used for child care include living/dining room, kitchen, bathroom 1, bedroom 1, and backyard. Off limits areas include bedrooms 2 & 3 which are inaccessible through use of child gate. The licensee has sufficient toys and equipment available. The home has a fenced backyard available for outdoor activities. LPA inspected new play structure installation completed on 12/12/23 per Licensee. New outdoor play structure, Lifetime model #1190453, is recommended for ages 3-12 was inspected and approved for use effective today. Licensee understands that supervision is required at all times during outdoor activities.
The fire extinguisher, smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were latched/locked and secured out of reach of children. There is no body of water on the property. Licensee states that there are no weapons in the home. First Aid and CPR certifications expire on 1/21/25 for Tamie Johnson-Schirlls and 1/14/25 for Amber Schirlls. Licensee has required immunizations. Licensees completed Mandated Reporter Training on 12/7/23 and is reminded it must be completed every 2 years. Children’s records were reviewed and found in order.
Provider is hereby reminded of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms; corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. All equipment that is used should be used only as intended by the manufacturer. |
