On 2/21/2025, at 9:15 a.m., Licensing Program Analyst (LPA), Cindy Meier conducted an unannounced Annual Inspection and met with Assistant, Erika Jones. Licensee, Karen Underwood was not present at the time of inspection. LPA disclosed the purpose of the inspection and was led on a tour of the facility indoors and outdoors. This facility is a one story, three-bedroom, one-bathroom house. The following areas are used for childcare: living room, playroom, and bathroom. Off limit areas include: dining room, kitchen, and bedrooms #1, #2, #3. Bedrooms are inaccessible through the use of door locks. The kitchen is considered off limits but does not include any barrier protecting children from entering. Hours of operation hours are Monday – Sunday, 23 hours. There were seven (7) children, and three (3) assistants present during the inspection.

The fire extinguisher, smoke detector and carbon monoxide detector met requirements. Hazardous items such as cleaning products, bucket of water, and knives were assessable to children in a kitchen lower cabinet, on the kitchen floor and in a kitchen drawer. LPA informed licensee poisons shall be placed in a storage area and locked. LPA did not observe any poisons during the inspection. LPA observed toys and materials available for children’s use. At this time the back yard is off limits and the children do not go outside for outdoor activities. LPA informed licensee to ensure children are supervised at all times during outdoor activities. There is no fireplace on the premises. Assistant stated there are no bodies of water and LPA did not observe any bodies of water during the inspection. Assistant stated there are no firearms, other weapons, or ammunition in the home.

A review of staff records on this date indicates that two (2) of the three (3) assistants present during the inspection have received criminal record and child abuse clearances or exemptions. One (1) assistant, A1, present during the inspection has not received a criminal clearance and stated she helps out occasionally.

LPA reviewed children’s files. Five (5) out of seven (7) children’s files reviewed were incomplete and did not contain the required documents.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Childcare Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Licensee’s and assistant’s Mandated Reporter AB1207 training were not available for review. Neither of the two (2) assistants had a current Pediatric CPR and First Aid certification card available for review. Licensee’s and two (2) assistants personnel files were not available for review. Facility roster is maintained and was reviewed. The licensee conducted and documented a fire and disaster drill on 9/24/2024. Required documents are posted. There is not one crib or play yard for each infant who is unable to climb out of the crib or play yard. Assistant, A1, stated the infant sleeps on the sofa or they hold the infant while it is sleeping. An LIC9227 Individual Sleeping Plan form and Safe Sleep logs were not available.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPA discussed the safe sleep regulations with licensee and discussed the Childcare Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA reviewed with licensee the LIC 311D, Forms/Records to Keep In Your Family Childcare Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

LPA and licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov. LPA discussed and provided Licensee with the following: childcare advocates email address: childcareadvocatesprogram@dss.ca.gov. Also, LPA Cindy Meier informed Licensee, Karen Underwood to provide a copy of this licensing report dated 2/21/2025 that documents Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
LPA will follow up with licensee, Karen Underwood.

During the exit interview, Licensee, Karen Underwood, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.
