On 9/3/26 at 11:15am, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced annual inspection with Licensee Cici Luo. Also present was the Licensee’s sister and helper, Zhuoya Luo.

The two-story home was toured and inspected to ensure the home provides a safe environment for the care and supervision of children. Eight day-care children were present during the inspection. The Licensee has provided adequate space for the children to eat, sleep, and play within the home. Areas used for childcare include the day-care room, bathroom #1, and backyard. Off-limits areas include the dining room, living room, garage, kitchen, bedroom #1, and bathroom located downstairs, as well as the entire upstairs area, consisting of four bedrooms and three bathrooms. The off-limits areas are made inaccessible through the use of safety gates. The stairs are properly barricaded. The Licensee has sufficient toys and equipment available for the children. The home also has a fenced backyard available for outdoor activities.

The fire extinguisher, smoke detector, and carbon monoxide detector meet requirements and are operational. All hazardous items were latched, locked, and secured out of the reach of children. There are no bodies of water on the property. The Licensee stated that there are no weapons in the home. The Licensee’s and helper’s First Aid and CPR certifications expire on January 25, 2027. The Licensee and helper have the required immunizations. The Licensee completed Mandated Reporter Training on December 8, 2024, and understands that the training must be renewed every two years. FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Children’s and staff records were reviewed and found to be in order. The LPA obtained an updated facility roster and observed that all required documents were posted in an area accessible to parents and guardians.The provider was reminded of the following: suspected child abuse and neglect must be reported; children’s records must be maintained in accordance with regulations; and all required forms must remain posted. Corporal punishment and smoking are prohibited. Exersaucers, bouncy seats, walkers, and jumpers are not permitted in the day care. All equipment must be used only as intended by the manufacturer.The LPA and Licensee discussed California Megan’s Law, and the LPA provided the following website: www.meganslaw.ca.gov.The LPA reviewed LIC 311D, Forms/Records to Keep in Your Family Child Care Home, with the Licensee, including children’s forms and records, facility forms and records, and information required to be posted.A review of staff records conducted on this date indicates that all facility staff and other individuals who require caregiver background checks have obtained the required criminal record and child abuse index clearances or exemptions.The Licensee was reminded that all adults 18 years of age or older who live or work in the home, including employees and volunteers, must obtain a criminal record clearance or exemption or transfer an existing clearance or exemption before their initial presence in a licensed family child care home. A civil penalty of $100 per day, per person, up to a maximum of $500 per person, will be assessed if this regulation is violated.The LPA discussed safe-sleep regulations with the Licensee and provided the Child Care Licensing Safe Sleep webpage as an additional resource: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep. The LPA also informed the Licensee of the importance of checking the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ for recalled infant equipment. The Licensee was encouraged to register all infant equipment with the CPSC to receive notifications of any recalls involving purchased equipment.The LPA verified that the Licensee is conducting and documenting the required 15-minute checks and has a completed and signed Individual Infant Sleeping Plan (LIC 9227) in the applicable children’s files. For general questions regarding licensing requirements, the Licensee may contact the Child Care Licensing Duty Line at (619) 767-2248.The Incidental Medical Services (IMS) policy was discussed. For IMS information, refer to the Evaluator Manual, Regulation Interpretations and Procedures for Family Child Care Homes, Section 102417. When IMS are provided, a Plan for Providing IMS must be submitted to the Department. The following Americans with Disabilities Act (ADA) information was provided: United States Department of Justice toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY). The publication Commonly Asked Questions About Child Care Centers and the ADA is available at http://www.ada.gov/childqanda.htm.No deficiencies were cited.An exit interview was conducted, and the report was reviewed with Licensee Cici Luo. A Notice of Site Visit was provided and must remain posted for 30 days.
