On July 23, 2024, at 1:55PM, Licensing Program Analyst (LPA), Luigi Gargaro, conducted an unannounced annual required inspection and met with licensee Haweya Shire and her cleared and associated daughter assistant, Hafsa Mohamed. LPA disclosed the purpose of the inspection and was granted entry into the facility by the licensee. Ms. Shire and seven (7) day care children were present as well as Ms. Mohamed during today's visit.

This facility is a two floor, four bedroom, three bathroom townhouse. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: the kitchen, the dining area, the living room, the first bedroom and the day care bathroom. Off limits areas are the second main floor bedroom, the second floor, the lower floor level which contains the home garage and the patio off the living room. The second bedroom is made off limits with a locking door handle. The upper and lower floors are made off limits with installed child safety gates at the staircase entrances. The patio is made off limits with a latching sliding patio door.

The fire extinguisher, smoke detector, and carbon monoxide detector met requirements. All hazardous items were inaccessible to children. The licensee has toys, play equipment and materials available. The licensee also has a patio off the first bedroom that is a supplemental outdoor play area for day care children. The patio has a wooden enclosure but as the enclosure does not completely go to the ground licensee understands that any time day care children are using the area, direct supervision must always be provided to ensure their safety and that they remain in the patio. Licensee also takes children to a local park for outdoor activity.

The complex has a gated community pool area that is maintained by the complex management and is only accessible with a facility gate key. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions.

Analyst provided licensee with a new blank safe sleep log and reviewed with her the requirement that all napping infants must be checked every 15 minutes and the finding for each 15 minute segment must be documented. Licensee states she will correctly document children's nap times until they turn two and send analyst a sample for children #1 and #2 for the period of 07/24/22-07/31/22 to complete the correction.

First Aid and CPR certifications expire in February of 2026 for the licensee. License and Ms. Mohamed have required immunizations. Licensee Shire is currently exempt from Mandated Reporter Training as English is not her primary language. Ms. Mohamed has a current Mandated Reporter Training certificate that she obtained on 01/24/24 Facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 11/17/23. There is one crib or play yard for each infant who is unable to climb out of the crib or play yard. Cribs or play yards are free from all loose articles and objects. The provider has not been physically checking on sleeping infants every 15 minutes. An Individual Infant Sleeping Plan [LIC 9227 (3/20)] is been maintained for each infant up to 12 months of age. The provider places infants up to 12 months of age on their backs for sleeping.

LPA provided and discussed the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms, and ensure that all adults residing or working in the home have criminal background clearances or exemptions. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensee was also provided handouts with information regarding upcoming Safe Sleep Regulations/SIDS, Lead exposure and Shaken Baby Syndrome. LPA and Licensee discussed California Megan's Law and LPA provided: www.meganslaw.ca.gov.

LPA discussed and provided Licensee with the following: child care advocates email address: childcareadvocatesprogram@dss.ca.gov . In addition, for general questions or questions regarding licensing requirements contact the Child Care Licensing Duty Line at (619) 767-2248.

Incidental Medical services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Two type B violations California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D.

An exit interview was conducted with the licensee. The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.

LPA provided a notice of site visit and advised licensee it is to be posted for 30 days in her facility notification area.
