13 | On 5/15/23 at 8:45 AM Licensing Program Analyst (LPA) Adrian Mangina conducted an unannounced inspection to deliver the findings for the complaint received 2/23/23. LPA met with Licensee Yezina Sallew. There were 2 children present. It was alleged that Licensees did not provide adequate supervision allowing one child to act inappropriately with another child. During the investigation, LPA completed document review, made observations, and conducted interviews of staff and others. LPA was unable to corroborate the allegation as there were conflicting statements from witnesses as to what exactly occurred. Licensee stated that the children were playing hide and seek in the same room when the incident occurred, and although she had the live feed showing in the room she was in, she did not see what happened as she was feeding other children in care, but she did hear one child make statements to another child to stop potential inappropriate behavior. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, therefore the above allegation is found to be Unsubstantiated. Exit interview conducted and report was reviewed with the Licensee Yezina Sallew. Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
