25 | On 4/17/23 at 11:10 AM Licensing program Analyst, (LPA) LPA arrived at the facility and conducted a case Management deficiency. LPA identified herself, showed her badge and was admitted to the home by Licensee Yezina Sallew. Upon arrival LPA Mangina observed at 11:10 AM that there were 4 infants and 2 toddlers in care. The facility was out of ratio. LPA was told that Licensee David Wang was at an appointment and that he left at approximately 11:00 AM. At 11:11 AM LPA observed that child #1 was in a bouncer seat. Licensee David Wang arrived at approximately 12:00 PM. LPA provided Staffing ratios and capacity regulations. LPA once again offered to refer to Technical Support Program and was refused.

LPA Mangina informed Licensees David Wang and Yezina Sallew that this report dated 4/17/23 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Mangina informed the facility representative to provide a copy of this licensing report dated 4/17/23 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the Licensees David Wang Yezina Sallew. The Licensees were provided a copy of this report. A Notice of Site Visit (LIC9213) was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
