On February 21, 2025 at 2:20 p.m., Licensing Program Analyst (LPA) Angela Nguyen conducted an unannounced Required-3 year Inspection and met with the Licensee, Euranika Carroll. LPA disclosed the purpose of the inspection and was granted facility entry by the Licensee. Present in the home is Licensee, assistant, Melissa Jose and six (6) day care children. Three (3) of those children were infants. This facility is a two (2) story, three (3) bedroom, one (1) loft and two (2) bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for childcare are: the art studio, downstairs bedroom and the fenced backyard. The off limits areas is the top floor and downstairs bathroom. The bottom of the stairs is barricaded with child safety gates.

The fire extinguisher, smoke detector, and carbon monoxide detector are operational. Hazardous items were observed in a locked cabinet inaccessible to children during this inspection. Children’s toys and play equipment are available and observed free of hazards. Licensee uses the fenced backyard for outdoor play. Licensee was reminded supervision is to be given to children whenever engaged in outdoor activities. Licensee stated there are no weapons in the home. LPA observed a fireplace with a safety screen in the home. Licensee stated there are no bodies of water. LPA observed no bodies of water. Licensee stated she provides two (2) meals and two (2) snacks to the children in care that includes all of the food groups.

Licensee’s First Aid and CPR certifications expire in 3/2026. Licensee’s and Assistant’s Mandated Reporter Training is expired at time of inspection. Assistant’s CPR/ First aide expire 9/2025. Licensee has required immunizations. LPA reminded licensee that Pediatric CPR/ First aid and Mandated Reporter Training expires every 2 years and shall be kept on file at the facility. The facility roster is maintained and was determined to be complete. Last fire and disaster drill was conducted and documented on 12/06/2024.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
Licensee stated Incidental Medical Services (IMS) policy was reviewed with licensee. Licensee stated she will contact LPA when there are children enrolled. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee and LPA discussed the facility emergency disaster plan, LIC 311D, safety of children and children’s file. LPA reminded Licensee of the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms. LPA obtained an updated LIC 279 from licensee. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Euranika Carroll, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile.

Type A deficiency was assessed during today's inspection, see LIC809D.

Type A deficiency which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. LPA informed the licensee to provide a copy of this licensing report dated 2/21/2025 that documents any Type A deficiency to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

(2) Type B California Code of Regulations, (Title 22, Division 12 & Chapter 3), are being cited on the attached LIC 809-D.

A copy of the report and appeal rights (LIC 9058) was provided to the Licensee and notice of site visit (LIC9213) was given to Licensee and was advised it must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
