On 6/15/21 at 12:50 PM, Licensing Program Analyst (LPA), Keturah Lane arrived at the facility to conduct a case management visit. Upon arrival, LPA met with Licensee Makisha McNeil. Present were the licensee, eight day-care children, and staff members Dejuana Givens and Tammy Davis. LPA inspected the physical plant to ensure the facility is in compliance with Title 22 regulations. There are no bodies of water in the home. Licensee and staff members were interviewed. Children were ages 2-3 years old and were not interviewed. There were no school age children present and all children were under the age of 5 years. LPA Lane discussed that Licensee was out of ratio. Licensee states her hours for child-care are Monday – Friday from 6:30 AM – 6PM. LPA obtained a facility Roster. Licensee uses the dining room, kitchen and backyard for dhild care. Off limits areas are living room, master bedroom, bedroom #1 and extra bedroom. The living room is inaccessible by use of a baby gate. Master bedroom has door locks but was accessible by outside screen door that leads to back yard play area at time of inspection. The other bedrooms were not locked and were accessible. At 1:15 PM, LPA observed body lotions, degreaser and body wash accessible in a storage closet in the hallway on a lower shelf. Licensee removed items to a higher shelf. LPA and Licensee discussed making the off-limit areas inaccessible.

Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiencies were cited (refer to LIC 809-D).

An exit interview was conducted with the Licensee. The Licensee was provided a copy of their appeal rights (LIC 9058) along with the report (LIC809) and their signature on this form acknowledges receipt of these rights. The LIC 9213 (Notice of Site visit) was posted during today's visit. Notice of Site Visit must remain posted for 30 days.

Licensee removed items to a higher shelf and will make all off-limit areas inaccessible by use of door locks or latches by 6/18/21 and send photos/video to LPA via e-mail.

Based upon LPA observation there were items (body wash, degreaser, disinfectant wipes) accessible in a closet on a medium-high shelf. Items were inside the closed closet. This poses a potential health and safety risk to children in care.
