On 11/3/21 at 11:30 AM, Licensing Program Analyst (LPA) Keturah Lane conducted an unannounced case management visit at the facility. Upon arrival, LPA Lane met with Licensee Makisha McNeil and toured the facility. The purpose of the visit was to follow up on an unusual incident and recent citations. When LPA arrived, the children were playing outside. There were 7 children in care at the facility. Children present were ages 2 & 3 years old. LPA Lane observed appropriate supervision and capacity, however Licensee was out of ratio since no school-age children were present. At 11:40 AM, LPA observed that the garage door was unlocked, master bedroom door was partially open and the bedroom next to the bathroom was unlocked (door was closed). The third bedroom was locked. LPA Lane reminded Licensee to keep the off-limit areas inaccessible by keeping the doors closed and locked. LPA Lane also observed the living room did not have a gate keeping it off-limits. Licensee stated she moved the gate from the living room to keep the sandbox off-limits to the children because they had been eating the sand. LPA Lane did observe a gate in the sandbox area upon arrival. Licensee stated she would move the gate back to the living room when the children return inside to keep the living room off-limits. LPA observed accessible toiletries (perfumes, makeup) within reach of children in the accessible off-limit areas. Licensee locked off-limit doors and corrected this at the inspection. One child was picked up at 12:13 PM, so at that time, Licensee was within ratio.

Pursuant to Title 22 of the CA Code of Regulations, the following Type B deficiencies were cited (refer to LIC 809-D).


Exit interview was conducted and report was reviewed with Licensee, Makisha McNeil. The Notice of Site Visit was provided and must remain posted for 30 days.

Based upon LPA observation, Licensee had 7 children in care and the criteria in Section 1597.44 of the Health and Safety Code was not met because the 7th child was not school-age or 6 years of age which is a potential health and safety risk to children in care.

Licensee corrected the deficiency immediately at the visit by locking the off-limit areas. LPA Lane reminded Licensee to make sure off-limit areas remain off-limits at all times when children are in care.
