On 7/16/2025 at 9:55 am, Licensing Program Analyst (LPA) Shannan Williams conducted an unannounced complaint inspection in regard to the above allegation. LPA met with Licensee Ceji Johnson and discussed the allegation. There were eight (8) children present at the facility.

The indoor and the outdoor of the home was inspected. Interviews were conducted with the licensee and three daycare children, Child 1 (C1) and Child 2 (C2) and Child 3 (C3). A facility roster was collected and children and staff records were reviewed.
Based on the Licensee’s admittance, on July 07, 2025, between approximately 10:30 a.m. and 11:30 a.m., Child #4 (C4) exited and re-entered the daycare facility’s transport van during unloading at the back of the facility without the knowledge of either Licensee's. The Licensee, Ceji stated that after Licensee, Sherri re-parking the van at the corner of Santa Elisabeth Ave. and Mount Bullion St., Child #4 remained unattended in the vehicle for at least 10 minutes.

The Licensee, Ceji stated that after Licensee, Sherri re-parking the van at the corner of Santa Elisabeth Ave. and Mount Bullion St., Child #4 remained unattended in the vehicle for at least 10 minutes.

Licensee Ceji Johnson stated, a neighbor came to the front door and stated to Licensee that they "saw a hand come up and press against the window of the vehicle from the inside and heard a child" and that, "they believed there was someone left the vehicle". Collaborating interviews with C1 and C2 who witnessed and recalled in detail the same event and supported the statements made by the Licensee.

LPA Shannan Williams informed licensee's Sherri and Ceji Johnson that this report dated 7/16/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is/are an immediate risk to the health, safety and personal rights of children in care.

Also, LPA S. Williams informed the Licensee to provide a copy of this licensing report dated 7/16/2025
LPA S. Williams informed licensee Ceji Johnson, that this report dated 7/16/2025 documents one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA S. Williams informed the licensee to provide a copy of this licensing report dated 07/16/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given to Licensee, Ceji Johnson, and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Licensee, Ceji Johson.

The Licensee has requested a referral to the TSP program and utilize that program to discover new and effecient ways to assist with documenting children, possible videos and training to assist. The Licensee will look into implimenting the brightwheel app to assist to with documenting where children are and

Based on Licensee admittance and interview, the licensee did not comply with the section cited above in that a child was left unattended in a vehicle without supervision which poses an immediate health, safety or personal rights risk to persons in care.

 physically moving them on the app to ensure an accurate head count. The Licensee will organize the children's schedules and figure out a better system to ensure a more solidified schedule for the children in care and will email this to LPA S. Williams at Shannan.williams@dss.ca.gov by 7/23/25
