This requirement was not met as evidenced by:	1 2 3 4 5 6 7	Licensee was advised that a child wandering from a day care home is not acceptable and a serious violation of Child Care regulations as well as a serious risk to the health and safety of a child.
	8 9 10 11 12 13 14	Based on interviews and record reviews, it was determined that a four year old child in care wandered from the facility on 10/19/23 before being found by his father at a neighbor's several houses down from the provider. Having a child elude the supervision of a day care provider without their notice is an immediate risk to their health and safety.	8 9 10 11 12 13 14	Licensee submitted a previous written plan of correction to prevent such future incidents that included installation of preventative equipment such as locks and alarms, home cameras and an increased supervision plan.

Licensee states she will submit a written statement to that effect to analyst to comfirm this and address the deficiency by 02/15/24.
	8 9 10 11 12 13 14	Based on confidential interviews and reviewed reports, it was determined that the Licensee was untruthful with the Department about two separate incidents in which the same four year old child wandered from her facility. This poses an immediate health and safety risk to children in care.	8 9 10 11 12 13 14	LPA also informed licensee that this information will be forwarded to the Department’s Legal Division for further review for an ongoing administrative action.
