On January 3, 2025, at 10:20 AM, Licensing Program Analyst (LPA), Luigi Gargaro, conducted an unannounced annual required inspection and met with the licensee, Patricia Tachiquin. LPA disclosed the purpose of the inspection and was granted entry into the facility by the Licensee. Five (5) children and one (1) staff member, Sonia Lopez Paredes as well as Ms. Tachiquin were present in the facility and caring for children during this inspection. This facility is a two floor, two bedroom, two bathroom house. Licensee accompanied LPA inside and out of the facility during this inspection. The following areas used for child care are: the kitchen, the dining/family room area and the master and second bathrooms all located on the main level of the home. The second day care room area located in the lower level of the home is also a for use room.

Off limits is the master bedroom also located on the main level of the home. The master bedroom is made off limits with a locking bolt on the sliding entrance door. While the lower level day care room is for use, the staircase accessing it is off limits and is made that way with child safety gates that are installed at the top and bottom of the staircase. In order to access the lower level day care room and back area of the home the licensee takes children through the right side alleyway. The lower level second bedroom, which was previously a for use bedroom, has been made into an off limits room and is made that way with an installed dead bolt lock and locking door handle. The licensee has sufficient toys and equipment available.

During today's visit licensee informed analyst that the lower level portion of the home was subjected to water damage due to a plumbing issue that occurred on 12/20/24. Analyst inspected the lower floor of the home during today's visit. The water has been removed and the licensee has been cleaning and/or replacing any water damaged rugs and rubber foam flooring for the play area.

However, the lower level of the home has been temporarily made off limits to the day care by the licensee and day care children will not be accessing it until the cleaning is complete and all the equipment and flooring have been put back in place or replaced if too damaged. Licensee states she estimates the cleaning and all necessary item replacement to be completed by the end of next week. Licensee states she will send analyst a photo of the lower level when it is ready so that he may review it and determine if it may be ready for use once more.

The fire extinguisher and combination smoke detector and carbon monoxide detector met requirements and are operational. All hazardous items were inaccessible to children. The licensee has toys, play equipment and materials available. The home has fenced front and back yard areas. The front yard is the primary outdoor play area for the facility. The side alley/driveway and back of the home area are just areas to access the lower level of the home and are not used as outdoor play areas. The licensee has an off limits lower yard behind the back home area that is made off limits with an installed fence and latching gate. The left hand side alley, which contains licensee's personal items, is made off limits with front and back latching gates.

Licensee also has two off limits lower level storage rooms that have entrance doors from the lower level day care room and are made off limits with locking door handles and door knob covers. Licensee has a child safety gate that is installed at her secondary side entrance door whenever the exit door may be left open as she has a five step staircase that leads from it to the side yard. No bodies of water observed on the premises during the inspection. Licensee stated there are no weapons in the home. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions.

Licensee’s First Aid and CPR certifications expire in June of 2025 and helpers Lopez-Paredes in January of 2025 with helper Navarro-Tachiquin in June of 2025. Licensee and assistants have required immunizations. Licensee stated she and assistants completed Mandated Reporter Training but did not have their PDF certificates available even though they had e-mails confirming the class. Facility roster is maintained and was reviewed. The last fire and disaster drills were conducted and documented on 09/20/24. Licensee currently has no infants in care but is aware of the safe sleep regulations if she ever is to enroll any infant children.

LPA provided and discussed the following: Report suspected child abuse and neglect, maintain children’s records according to regulation, post all required forms, and ensure that all adults residing or working in the home have criminal background clearances or exemptions. Licensee was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers and/or similar equipment are not allowed in daycare. Licensee was also provided handouts with information regarding upcoming Safe Sleep Regulations/SIDS, Lead exposure and Shaken Baby Syndrome. No deficiencies cited were cited during today's visit.

An exit interview was conducted with the licensee, Patricia Tachiquin. The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.

A notice of site visit was provided by the LPA and must remain posted for 30 days.
