32 | them the following week; however, parents interviewed reported that they do not take the cups home for cleaning.
Additionally, it was determined that staff do not implement proper infant sleep practices by not maintaining one (1) crib or play yard for each infant in care. On 04/10/2025, during inspection, LPA observed three (3) infants present with only two (2) available play yards. Staff interviewed stated the additional infant would sleep in a stationary “rockaRoo” on a cot or mat instead. Witnesses interviewed indicated that they had seen infants sleeping in rockaRoos with blankets on multiple occasions.
Furthermore, the licensee did not ensure proper supervision was provided to daycare Child #2 (C2) on multiple occasions. On 04/10/2025, during inspection, LPA observed C2 unsupervised outside in the backyard, playing with rainwater pooled in a play kitchen sink. C2 was observed wetting his hands, bringing them to his mouth, and attempting to drink the water. C2 was unsupervised for approximately 10 minutes until the licensee took C2 back inside the home. Witnesses interviewed confirmed that it was common for C2 to be outside unsupervised, often attempting or putting things in their mouth.
Based LPA observations and interviews conducted, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Per California Code of Regulations (Title 22, division 12 & Chapter 3) 3 Type A citations are being cited on the attached 9099-D page.
LPA Julieta Abrego informed Licensee, Patricia Pavia that this report dated 07/01/25 document(s) three (3) Type A citation(s) which shall be posted for 30 consecutive days as there is immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Julieta Abrego informed Licensee, Patricia Pavia to provide a copy of this licensing report dated 07/01/2025 that documents Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
