25 | On April 3rd, 2024 at 10:45 AM, Licensing Program Analyst (LPA), Jo Ann Legaspi conducted an unannounced capacity increase inspection. Licensee Rosa Juarez was advised of the meeting’s purpose and granted LPA facility entry. Present in the home was the Licensee, Wilber Gonzalez Tobar (background cleared spouse) and eight (8) children. Wilber Gonzalrez Tobar provided Spanish language translation services.
On 02/28/2024, Licensee submitted an application (LIC 279) requesting a capacity increase. The Fire Safety Inspection Request (STD 850) was approved by the local fire marshal on 03/21/2024 for fourteen (14) children. Landlord Consent is on file.
This two (2) story, five (5) bedroom, three (3) bathroom house was toured and inspected. The following areas used for childcare are: the living room, family room, enclosed patio room, one (1) bathroom, fenced backyard and fenced front yard. The off-limits areas include the kitchen, remaining bathrooms, and bedrooms. Kitchen cabinets have sliding cabinet locks. The stairs to the upstairs bedrooms and remaining bathrooms are gated with a retractable child safety gate. Licensee accompanied LPA on a tour of the home, as shown on the updated facility sketch. Background criminal record clearances were verified and discussed. First Aid and CPR certifications expire in May 2025. Mandated Reporter Training was completed on 03/06/2024. Facility has working 2A10BC fire extinguisher, smoke alarms, carbon monoxide, and the first aid kit in place. The last safety drill was on 12/19/2023. There are no bodies of water on the premises. The Licensee states no firearms/ammunition are housed in the facility. There is a small dog in the home. The daycare schedule is weekdays 6 AM to 6 PM.
The Licensee provided proof of control of property. Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the licensee confirms was provided to the property owner/landlord. The licensee obtained a signed |
