13 | On June 5, 2026 at 10:30AM, Licensing Program Analyst (LPA), Adrian Castellon conducted an unannounced complaint inspection to deliver the finding for the above listed allegation. LPA met with Licensee, Marian Hernandez Flores, and advised licensee of the purpose of the inspection and conducted a tour of the home. It was alleged that licensee does not reside in the home. During the course of the investigation, interviews were conducted with the Licensee, reporting party, daycare parents, two persons who reside in the home, and a facility assistant. LPA counducted surveillance. Licensee states that she does live in the home. LPA obtained docuements pertaining to the investigation (mortgage statement, rent payment, credit card statement). Due to conflicting information obtained throughout the course of the investigation and no other witnesses to the alleged allegation, LPA was unable to determine whether or not the allegation occurred. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Exit interview conducted and report was reviewed with the Licensee, Marian Hernandez Flores. A Notice of Site visit was given and must remain posted for 30 days. |
