On 2/10/25 at 9:15 AM, Licensing Program Analysts (LPAs) Keturah Lane and Hanna Lucas visited the facility to conduct an annual inspection. Upon arrival LPA met with Director Naomi Shakhman and Assistant Director Romy Rothstein and proceeded to tour the facility. During today's inspection, there were 30 infants/toddlers in care and LPA observed the following ratios:

Infant room (#155 - Caterpillars) had 5 infants (0-12 months) w/staff members Rachel Dolin, Sofia Dujowich and Jocelyn Silva
Room #154 (Butterflies) (9 months – 18 months) had 5 infants w/staff members Maria Guiterres and Susha Ariyathilake
Room #124 (Red Ladybugs) (18 – 24 months) had 10 toddlers w/staff members Shani Parsha, Erika Schottland, Chansomale Kun and Bertha “Lucy” Narvarez
Room #125 (Orange Ladybugs) had 10 toddlers with staff members . Lillian Boaron, Maria Espinoza, Miriam Delgado and Sandra Guiterres

Appropriate ratios, capacity, care and visual supervision were observed by LPAs during the inspection.

Rooms have adequate heating, lighting, ventilation. Floors appear to be clean and safe. Children bring their lunch to school and supply their own snacks. There are extra snacks available in the office. Cleaning supplies are kept separate from food and are inaccessible to children. Storage containers for solid waste have tight-fitting covers and are kept in good repair. There is an armed guard on the premise and facility is in process of applying for a waiver. All foods/beverages capable of rapid spoiling are stored in covered containers at 45 F or less. The facility appears to be free of insects and rodents. There is an operational carbon monoxide detector at the facility. Last emergency drill (fire) was conducted on 12/16/24. (continued on LIC809-C...)

All infant furniture & equipment is age appropriate and in good repair. LPAs observed several walkers in two of the classrooms and one exersaucer (and more walkers) outside on the playground. High chairs or feeding tables have broad-based legs, plastic seats in good repair, and tray locked. Hazardous materials are inaccessible to children. Bottles, dishes and containers of food brought by infant’s authorized representative are labeled with the infant’s name and current date. The changing table is within arm's reach of a sink. There is no potty-training in any of the infant/toddler rooms. Caregivers wash hands before and after each feeding and each diaper change. Toys are safe, with no sharp edges, splinters or protruding points, nor small parts that can be pulled off and swallowed. Floor mats and cots are available for infants able to climb out of cribs. Cribs or napping equipment are available. LPAs observed one crib with a sheet that was not tight-fitting and advised teachers to obtain a sheet that is the correct size from the parents for the child’s mattress. Bedding is identified and stored separately for each infant. Napping infants are under visual observation by staff. Indoor activity space is physically separate from other components. Facility maintains individual feeding plans for each infant and has up to date Infant Needs and Services Plans for each infant on file. Sleep logs were reviewed. LPAs observed some LIC9227 Individual Infant Sleeping Forms that needed updating or were filled out incorrectly and offered technical assistance.

Outdoor play area is a fenced playground with sufficient material for cushioning. There are no bodies of water at this facility. Climbing structures, swings and slides are securely fixed to the ground. Area has trees and canopies used for shade. Equipment is age appropriate. Area has drinking water readily accessible, and grounds are free of debris or potential hazards. LPAs observed an area that was blocked with a sand table where the ground sloped a bit. The sand table had another table on top of it with leaves and debris which was removed. Director stated she would talk to maintenance about another way to block the sloping area of the ground. LPA also discussed with Director the cleaning procedures for outdoor toys and equipment as LPAs observed some of the toys/equipment were visibly dirty and needed cleaning.

Director stated there is no medication currently for any infants/toddlers and if so, it is stored in the office. Personnel records contain health screening documentation and at least one staff member has current CPR and First Aid certification. All staff have completed mandated reporter training and required immunizations. Infant care teachers meet educational requirements. Sign ins were reviewed and are through the Brightwheel app. Children’s records contain admission agreements and medical assessment. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
