13 | On 6/26/25 at 1:00PM, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding the above allegations. LPA Sutherland met with Director Ursula Jenkins and toured the facility. Census was 30 children and 8 staff members. The Department fully investigated the above allegations. LPA toured and observed multiple staff members in the classrooms having open play time. Serious injuries are reported via phone at time of injury and a report available at pick up. The classroom was fully staffed when these incidents allegedly occurred. The infant in question is learning to walk and has become more active. LPA reviewed children files and found children in the mobile classes to have more ouch reports than other classrooms. LPA conducted interviews with staff members and parents and there we no corroborating reports of concerns about supervision or injuries. Although LPA did not find evidence of the allegation during the investigation, it cannot be conclusively proven or disproven an injury or injuries occurred could have been prevented. Therefore, it is determined to be Unsubstantiated. A finding of Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violation occurred. An exit interview was conducted with the Director. A Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) was provided along with the report (LIC9099) to the Licensee. LPA Sutherland observed Notice of Site Visit being posted. Notice of Site Visit must be posted for 30 days.
