13 | On 3/20/26 at 12:00PM, Licensing Program Analyst (LPA) Annette Sutherland conducted an unannounced complaint inspection to deliver findings regarding the above allegations. LPA met with Director Ursula Jenkins and toured the facility. The census at the time of inspection was 25 infants and 7 staff members. Interviews with staff in the infant rooms indicated that they are knowledgeable about safe sleep protocols and reported adhering to them. The investigation also addressed the allegation that an infant had been left in soiled diapers and clothing for an extended period. Documentation reviewed on the facility’s app used to document daily activity, indicated that a diaper change entry was missed; however, staff reported this was due to an error and stated that the diaper was changed. No similar concerns were voiced during parent and staff interviews, there is no video available nor witnesses to the reported incidents. The Department conducted a thorough investigation of the above allegations and concluded that the information obtained neither proves nor disproves that the allegations occurred. Therefore, they are determined to be Unsubstantiated. An exit interview was conducted with the Director. A Notice of Site Visit (LIC9213) and Appeal Rights (LIC9058) were provided along with the report (LIC9099) to the Director. LPA observed the Notice of Site Visit being posted. The Notice of Site Visit must remain posted for 30 days. |
