25 | On 03/12/25, Licensing Program Analyst (LPA), Kelli Waters, arrived unannounced for an annual inspection. While at the facility, LPA Waters conducted a Case Management visit to follow up on two Unusual Incident Reports (UIRs) that were submitted to Licensing by the facility on 12/18/24, regarding an incident that took place regarding an employee (S1). LPA met with Center Director, Maria Ballesteros to discuss incident.
The Licensee, Quality Children’s Services (QCS), submitted reports regarding the following; S1 informed QCS of a personal legal situation as well as court documents pertaining to that of the legal matter. QCS contacted Community Care Licensing to confirm the clearance status of S1 and the steps QCS needed to take to remain in compliance with Title 22 regulations.
During the visit, Director Ballesteros and QCS human resources were able to confirm that S1 was no longer employed by the facility as a teacher assistant as of 12/26/24, but does remain a substitute cook with QCS as of 01/02/25. Director Ballesteros stated the center does not transport children and that S1 did not transport children as part of her duties while working at the center. LPA Waters was able to confirm for QCS that S1 still has a cleared background status through Guardian and is able to work in a childcare center as of this date. LPA Waters determined that the facility took the necessary steps to report this information and comply with Title 22 regulations.
An exit interview was conducted, and a copy of this report was provided. |
