32 | Regarding the allegation that staff placed infants on their stomachs to sleep, LPA observed infants sleeping in cribs positioned on their stomachs; however, facility records confirmed that LIC 9227 Infant Sleep Position forms were on file documenting that the infants were able to roll over independently. Therefore, this allegation was Unsubstantiated.
Regarding the allegation that staff falsified sleep logs for daycare infants, LPA was unable to obtain evidence to corroborate that sleep logs were falsified. However, during the investigation LPA observed that sleep logs were not being maintained for infants who were commingled in the adjacent preschool classroom, and a citation was issued on a case management. Therefore, this allegation is Unsubstantiated.
Regarding the allegation that staff placed pillows on sleeping daycare infants, based on observations and interviews conducted during the investigation, LPA did not observe pillows in the infant sleeping area or on sleeping infants, nor were there any interviews conducted that corroborated the allegation. Therefore, this allegation was Unsubstantiated.
Based on information obtained during interviews, observations, and documents reviewed, the allegations could not be corroborated. Although the allegations may have happened or may be valid, there is not a preponderance of evidence to prove that the alleged violations occurred; therefore, the allegations are determined to be Unsubstantiated.
An exit interview was conducted, and this report was reviewed with Facility Director Amitha Perusinghe. A Notice of Site Visit was provided and must remain posted for 30 days. Appeal rights were discussed and provided. |
