On 4/15/2026 at 10:20 AM, LPA Mahjoba Mohsini conducted an unannounced complaint visit for the complaint received on 2/4/26 for the purpose of delivering findings on the above referenced allegations. Upon arrival, LPA met with the the director Amitha Perusinghe. A tour of the facility was conducted, and LPA observed 29 children with 5 staff as follows:

Room 3: 18 children with 2 staff: Maria Morales, and Fernanda Morales (Aide)
Room 4: 11 children with 3 staff: Priyadharshani Wickramaratne, Lorena Martinez (Aide) and Kelly Barcellos

It was alleged that Facility operates out of ratio. Based on interviews, review of attendance records, staffing schedules, and observations made during the visits, the allegation was not corroborated. Staff, parents and children expressed no concerns regarding supervision levels or staffing. Although commingling was observed and cited, no evidence indicated that the facility was operating out of ratio. Therefore, the allegation is unsubstantiated.

It was alleged that Child sustained injuries due to lack of supervision. Based on Interviews with staff, parents and children, as well as a review of incident documentation, the allegation was not corroborated. No reports or observations suggested injuries resulting from inadequate supervision. Minor peer-to-peer incidents were described as typical for the age group, with no pattern indicating neglect or unsafe practices. Therefore, the allegation is unsubstantiated.

It was alleged that staff do not serve quality food to meet the needs of the children. Interviews with staff, parents, children and observations conducted during the course of the investigation did not support this allegation. Children reported enjoying a variety of meals, and LPA observed normal meal participation, including requests for additional servings. Staff reported that children’s food preferences vary, but no concerns were identified regarding food quality or adequacy. The review of menus showed meals consistent with program requirements. Therefore, the allegation is unsubstantiated.

Although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred, therefore the above allegations are found to be Unsubstantiated.

Exit interview conducted and report was reviewed with the director Amitha Perusinghe. A notice of site visit was given and must remain posted for 30 days
