On 05/12/2023 at 11:40am, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. Upon arrival, LPA met with Kristie Spillane/Preschool Operations Supervisor on campus and proceeded to tour the facility located in room Building E 10. The facility currently operates Mon-Fri from 7:30am to 5:30pm. There were 21 children in care supervised by Director Jana Conger, teachers Allison Praisner and Erlinda Stephens. The children were observed sitting at the rug and taking turns to wash there hands before they eat lunch. Facility is within staffing ratio. All required notices, forms and license were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water from an operational water fountain. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cots which are kept in classroom and each cot has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Menu is posted. Adequate food is available for snacks in the refrigerator located in the classroom. Cleaning supplies are kept are kept out of reach of children. Outdoor play area is a fenced playground with sufficient rubber cushioning. Climbing structures and slides are securely fixed to the ground. Area has a large shade structure at the playground. Equipment is age appropriate for the preschool. Outdoor area has an operational drinking fountain and grounds are free of debris or potential hazards. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the area by the teacher's desk or outside. All personnel and five children's records were reviewed, and staff members have the required immunizations and completed the mandated reporter training. Reporting requirements was also reviewed. All personnel have required criminal record and child abuse index clearances or exemptions and are associated to the PUSD Rolling Hills Administrative Site. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 04/24/2023. Facility has an operating carbon monoxide detector. Facility has at least one staff member that has a valid EMSA approved CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.
