On 06/28/2023 at 12:20PM, Licensing Program Analyst (LPA) Selina Siao conduct an annual inspection. Upon arrival, LPA met with Director Tobi Steiner and proceeded to tour the facility. There were 88 children in care during nap time supervised by 11 staff members. Facility is within staffing ratio. All required notices, forms and licenses were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water are available for the children. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of mats which are kept in the classroom closets. Each mat has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Children brings their own lunch to school and the facility provides snacks for the children. Snack menu is posted. Adequate food is available for snacks. Cleaning supplies are kept out of reach of children. Outdoor play area is a fenced playground with sufficient wood chips for cushioning under and around the high climbing structure. Climbing structures and slides are securely fixed to the ground. Area has ample large umbrellas structures for shades. Equipment is age appropriate and separated by age groups infant and preschool. Children brings their own water bottles to school and it is available to them when they are inside and outside at the playground. There are drinking water available for the children to refill their own water bottle or have the teacher refill their bottles. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the Director's office.

Administration of Child Day Care Licensing
(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Based on record review, the facility did not comply with the section cited above as two out of six staff members online mandated child abuse training certificate had expired. One of the staff's online mandated child abuse training completion did not show her name. This poses/posed a potential health, safety or personal rights risk to persons in care.

A sample of personnel and client records were reviewed, and staff members have the required immunizations. Not all staff members have completed the online mandated reporter training. All personnel have the required background clearances and are associated to the facility. LPA reviewed Emergency Disaster Plan and the last fire drill was conducted on 02/01/2023. Facility has an operating carbon monoxide detector in each classroom. Facility has at least one staff member that has a valid EMSA approved pediatric CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Refer to the next page LIC 809D for type B citation issue. An exit interview was conducted, and appeal rights were provided.

A notice of site visit was given and must remain posted for 30 days. Failure to keep notice posted will result in a civil penalty of $100.00.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov.
