On 07/08/2025 at 12:30PM Licensing Program Analyst (LPA) Hanna Lucas, made an unannounced visit to deliver findings on the above allegation. LPA conducted the initial complaint inspection on 06/26/2025.There were 86 children and 13 staff at the facility today.

Throughout the investigation, LPA confirmed through staff interviews and a file review that a staff member(S1) was smoking inside the classroom’s restroom, and the hallway of the facility. (S1) received a corrective action from the Principal on 05/19/2025 that stated, “On April 24th, (S1) was observed vaping outside the bathroom in the center…”.

The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. A TYPE A deficiency will be cited on the attached LIC 9099-D.

(S1) and (S2) confirmed that they were aides, and have been left alone with children at various times, in multiple classrooms. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. A TYPE B deficiency will be cited on the attached LIC 9099-D.

An exit interview was conducted with the Assistant Principal, Rocio Sainz. Appeal rights were provided and discussed. A Notice of Site Visit was provided and must remain posted in public view for 30 days

Assistant Prinicipal, Rocio Sainz, stated that she will ensure that no aides are left alone supervising children. Miss Sainz also stated that she will provide unqualified staff resources on how to get reimbursed for pursing the required education to be a fully quailfied teacher. LPA confirmed that no aides were supervising children alone at the time of the inspection.

Based on file review and staff interviews, it was found that (S1) and (S2) supervised children alone at various times, in multiple classrooms, without the direct supervision of a fully qualified teacher. This poses/posed a potential health, safety or personal rights risk to the children in care.

LPA informed the Assistant Principal, Rocio Sainz, that this report dated 07/08/2025 documents 1 TYPE A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.

LPA explained that the facility is required to provide a copy of this licensing report to the parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to parents/guardians of any newly enrolled child for next 12 months from the date of this report.

A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in each child's file for verification.

An exit interview was conducted with the Assistant Principal, Rocio Sainz. Appeal rights were provided and discussed. A Notice of Site Visit was provided and must remain posted in public view for 30 days.

Assistant Principal, Rocio Sainz, will be conducting a training with all of the staff members on 07/17/2025. Miss Sainz will provide a copy of the training agenda and a sign in/out sheet with teacher signatures, verifying that were present for the training, to the LPA by 07/24/2025.
