On 08/24/2023 at 4:30 PM, LPA Dana Stevens, conducted an unannounced complaint visit to deliver findings on the above allegation. LPA met with Director, Sandra Sandoval and informed her of the purpose of the visit. There were 8 children present with 3 staff at the time of the inspection.

During the investigation LPA conducted two unannounced inspections of the facility, interviewed staff, children, and daycare parents, and reviewed facility records.

Information obtained from interviews and record review revealed that although a physical altercation did occur between two students on 05/09/2023, the facility was found to be in substantial compliance with child/staff ratios at the time of the incident and the altercation was observed by the supervising staff who attempted to intervene, thus the allegation Facility staff failed to adequately supervise children in care resulting in physical altercation(s) is deemed, Unsubstantiated. A finding that the complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

No Deficiencies cited.

Exit Interview conducted and copy of this report and appeal rights provided to Director. Notice of Site visit mudt be posted for 30 days.
