On 04/25/2023 at 11:31am, Licensing Program Analyst (LPA) Selina Siao conduct an unannounced annual inspection. Upon arrival, LPA met with Assistant Director Diana Evangelista and proceeded to tour the facility. The following ratios were observed: Room 7 had 9 children supervised by teacher Socorro Hernandez, Room 8 had 10 children supervised by teacher Sarah Fuhr aka Bennett, Room 3 had 10 children supervised by Carrie Wilke, Room 2 had 11 children supervised by teacher Sarai Cruz and Room 1 had 10 children supervised by teacher Patricia Gonzalez. Facility is within staffing ratio during LPA's tour.

All required notices, forms and license(s) were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water from an operational water fountain. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cots which are kept in the classrooms and each cot has its own sheet or covering. Bathrooms are maintained with operational toilets and faucets with appropriate temperature. Paper towels and toilet paper are available. Bathroom is lighted and has ventilation. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals and snacks. Outdoor play area is a fenced playground with sufficient sand for cushioning. Climbing structures and slides are securely fixed to the ground. Area has two large trees use as shade. Equipment is age appropriate and separated by older and younger preschool children. Area has an operational drinking fountain and grounds are free of debris or potential hazards. LPA reviewed sign in sheets, first aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the Director's office. Personnel and client records were reviewed, and staff members have the required immunizations and completed the online mandated child abuse training.

Reporting requirements was also reviewed. All personnel have required criminal record and child abuse index clearances or exemptions. LPA reviewed Emergency Disaster Plan and last fire drill was conducted on 09/2022 which is more than six months ago. Facility has an operating carbon monoxide detector located in the kitchen. Facility has at least one staff member that has a valid EMSA approved CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

To improve the quality and value of the new inspection process, a survey will be sent to the email address provided. Please complete the survey and share your inspection experience. Facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on LPA Siao's observation, the licensee did not comply with the section cited above as room 7 had cleaning supplies under the sink cabinet that are accessible to children. Which poses/posed a potential health, safety or personal rights risk to persons in care.
