This is an amended version of the original report created on 3/17/23.
On 3/17/23 at 4:00pm, Licensing Program Analyst (LPA), Martha Malane arrived at the facility to conduct a case management inspection for the purpose of a capacity increase and room changes to remove room 112 and add room 125 to the preschool license. Upon arrival, LPA met with Director, Delisha Doakes and was led on a tour the facility. There were 59 children and nine (9) staff members present. The facility is requesting a capacity increase to 148 preschool children (age 2-5 years) in rooms 102, 103, 104, 105, 113, 114, 115 and 125. Hours of operation are Monday through Friday 7:00am to 5:30pm.

The indoor space measured a total of 5078.67 square feet which is sufficient to accommodate 145 children. However, applicant is requesting a capacity of 148 children. Classrooms are equipped with appropriate furniture, including mats for napping, tables, chairs, cubbies, bookshelves and other materials for children. The facility has a total of 12 toilets and 12 sinks exclusive for the children's use, which is sufficient to accommodate the requested capacity.

The outdoor activity space was inspected and measured a total of 2199.76 square feet which is sufficient to accommodate 29 children. The outdoor activity space area utilized by Elementary School students was inspected and measured a total of 25352.53 which is sufficient to accommodate 338 children. The total outdoor activity space measured 27552.29 square which is sufficient to accommodate 367 children.

A fire clearance for 148 preschool children (age 2-5 years) was granted 3/13/23.


The following is required:
1. Waiver request submitted to the SDRO for the outdoor activity space
2. Updated LIC200A
No deficiencies cited. Exit interview conduct with Director, Delisha Doakes. Notice of Site Visit shall be posted for 30 days from today's date. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
