Licensing Program Analysts (LPA), Jessica Rubio and Anastasia Flores conducted an annual inspection as part of a compliance review. This is a combination childcare center and the other licensed programs are: Preschool and School Age. The school age program was also inspected on this date. The preschool program was not inspected on this date. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

· The following items were posted and updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Menu
· The facility is operating with the limits as stated on the license.
· Ratios are being met during this inspection. LPA’s observed 10 children in care with one teacher and two aids providing care and supervision
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards.
· There are no weapons present at the facility as stated by Assistant Director Evelyn Avila.
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Drinking water is provided in the indoor and outdoor activity space by filtered water supply.
· Medications are stored where inaccessible to children in a locked cabinet in an office.

· Hazardous items were not stored where inaccessible to children. LPA Rubio observed several sharp scissors in an unlocked drawer in the infant/toddler classroom. A citation will be issued.
· Disinfectants, cleaning solutions and other items that are dangerous were stored inaccessible.
· Poisons and toxins are locked and inaccessible to children.
· All floors were observed to be clean and safe
· Highchairs and tables have broad-based legs and trays lock into place
· Changing tables are within arm’s reach of a sink; padding is at least 1” think with raised sides of at least 3” covered in washable vinyl or plastic
· Hand washing is completed before and after each diaper change and before feeding an infant
· Bathrooms were observed to be safe, sanitary and in operating condition; there is at least 1 potty chair for every 5 potty-training infants
· Playgrounds are enclosed by appropriate fencing. LPAs observed shrubs on the outdoor playground perimeter fencing to be growing into the play area and also contained several bees. A technical violation will be issued.
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed and meets regulation requirements.
· Disaster drills are conducted at least every six months – last drill conducted on 7/12/2022

At 10:43 am, a review of staff and children's records were conducted as part of this evaluation.
· Ten children’s records were reviewed. One child (Ref #1) was missing a TB test/risk assessment. One child (Ref#2) was missing a physician’s report, TB test/risk assessment and immunizations. Ten children were missing sleep logs. Citations will be issued.

· Four staff records were reviewed. Four out of four staff were missing or had expired Mandated Reporter Training. A citation will be issued
· Two staff (Ref#3 & 4) were missing physician’s report, TB test and immunizations. Citations will be issued.
· A staff member is present with current Pediatric CPR/First Aid which expires on 6/4/2024
· Opening and closing staff member’s (Assistant Director) CPR/First Aid expires on 6/4/2024
· Assistant Director completed Health and Safety Training in Summer 2016
· All staff present meet minimum qualifications for the position for which they were hired.
· A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. LPA discussed the safe sleep regulations with Assistant Director discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee Assistant Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assistant Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
