25 | On 07/31/2024 at 9:30 am, Licensing Program Analysts (LPA) Kelly Gerth and Sumayya Habeebulla arrived unannounced at the facility and met with Facility Representative Evelyn Avila to conduct a case management visit. During the tour of the facility, LPA’s observed that a total of 41 school age students were present in the combined front school age classrooms. As per the facility sketch with the department, there are two separate classrooms. Facility Representative, Evelyn Avila stated they use the two classrooms as one and therefore there is one fully qualified teacher and 2 aides present. LPAs reviewed the regulation with the facility representative and informed the facility that in order to have 41 school age children present at the facility, there must be 2 fully qualified teachers and 1 aide. LPAs provided a copy of the regulation requirement to Facility rep.
LPA s also observed there were 23 school age children present in the section Krystal’s Kiddos classroom with one fully qualified staff and 18 school age children present in the sea otters section with 2 aides. LPAs advised Facility Representative, Evelyn Avila that the teacher cannot be supervising more than 14 children at a time. Facility representative moved one aide to the section of Krysta’s Kiddos along with 4 school age children during the tour.

Appeal Rights were discussed and provided to Facility Representative, Evelyn Avila
A Notice of Site visit was given, Facility Representative, Evelyn Avila was reminded the notice must remain posted for 30 days.
