On 9/19/22 at 10:55am, Licensing Program Analyst (LPA), Martha Malane conducted a case management inspection regarding a self reported incident on 9/6/22 in which child 1 (C1) was released from care without supervision of an authorized person. Upon arrival, LPA met with Lead Teachers, Ireri Caro and Aida Bravo and was led on a tour of the facility. There were no children present as it was break time between the AM and PM sessions. The AM session operates 8:00am-11:00am and the PM session operates 12:00pm-3:00pm.
During the course of the incident investigation, interviews were conducted with staff and parent. The campus was toured and Lead Teacher, Ireri Caro led LPA through the area C1 walked without supervision. Based on information gathered, it was determined that on the day of the incident, 9/6/22, staff 1 (S1) and staff 2 (S2) released C1 from care without supervision of an authorized person. C1 walked through the preschool playground, out the gate, down the ramp and through the gate leading to parking lot. When C1 was exiting the gate to the parking lot their authorized repetitive arrived and observed C1 unsupervised. C1 currently attends the facility. See LIC809D for deficiency cited.
LPA, Martha Malane informed Lead Teacher, Aida Bravo, that this report dated 9/19/22 documents one (1) Type A citation which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care. Also, LPA, Martha Malane informed Lead Teacher, Aida Bravo to provide a copy of this licensing report dated 9/19/22 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview was conducted with Lead Teacher, Aida Bravo. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
