25 | On 12/06/24 at 10:00AM, Licensing Program Analyst (LPA) Luigi Gargaro conducted a follow up case management visit regarding a self-reported 10/30/24 lack of supervision incident in which child #1 (C1) walked out of her preschool classroom (Room 3) and walked to the library without being observed by the classroom staff.
During the course of the incident investigation, analyst conducted interviews with classroom staff and the school librarian and administrator. Analyst also reviewed related documentation regarding the incident and reviewed area where eloping child walked after leaving classroom.
Based on information gathered, analyst determined that on day of incident staff members #1, #2, #3, #4 and #5 (S1, S2, S3, S4 and S5) were either in the classroom or immediately outside supervising a total of nineteen day care children. At the time of the incident, C1 managed to push open the back door of the classroom, escaping the notice of both the indoor and outdoor staffers. The classroom safety feature of a door chime that rings when the door is opened was not engaged at the time the child opened the door.
The incident appeared to have occurred when the three indoor staff members were engaged in activities with children in the classroom, the outdoor staff member was engaged with children conducting outdoor activities and a fifth staff member was in the school office. Not being seen exiting the classroom, C1 left the preschool classroom area and walked across the plaza to the main elementary school area before entering the elementary school library without being noticed as missing and passing potential hazards such as the school elevator, staircase and restrooms.
Once in the library, C1 was discovered by the librarian, after being there multiple minutes. The librarian determined the child was a preschooler and contacted the elementary school administrator who then contacted the preschool who, at the time, still were unaware the child was missing. C1 was witnessed as crying by staffers who found her until she was calmed down by them but otherwise suffered no injuries. It was determined that the child was without supervision for nearly seven minutes and that as many as twelve minutes passed before the classroom staff was made aware that the child had left the classroom. |
