On 8/27/2025 at 9:00AM Licensing Program Analyst(LPA), Mahjoba Mohsini, made an unannounced visit to deliver finding on the above referenced allegations. LPA met with Director/Principal Camille Stern. LPA toured the facility and observed 47 children with 16 staff in 4 classrooms. The facility is within ratio and capacity.
During the investigation, LPA conducted observation, interviewed staff and reviewed relevant documentation. The information obtained shows that there were sufficient corroborating statements to show that Child #1 (C1) was physically pulled to the restroom by the hand after refusing several times. This is a violation of a child’s personal rights. Additionally, interview statements confirm that children were directed to use disinfecting spray to wipe their nap mats. The bottle states 'keep out of reach of children,' and is considered a hazardous item accessible to children.
The preponderance of evidence standard has been met; therefore, the allegations are considered Substantiated. Type B deficiencies will be cited on the accompanying LIC 9099D.
Exit interview was conducted and report was reviewed with Director/Principal Camille Stern. Notice of Site visit posted during this visit and must remain posted for 30 days.

Mrs. Stern stated the facility has implemented ongoing staff trainings. As part of continued professional development, Mrs. Stern will conduct staff training on children’s personal rights and will provide proof of completion to the LPA via email by 09/30/2025.

Mrs. Stern stated staff will no longer allow children to handle disinfectants. All disinfectant sprays and similar cleaning products will be stored in an inaccessible area at all times.

continued...Staff will be retrained on the safe storage and use of cleaning products, including disinfectants and pesticides. Proof of staff training will be submitted to LPA via email by 09/30/2025.
Failure to correct the cited deficiency(ies), on or before the Plan of Correction (POC) due date, may result in a civil penalty assessment.

On 8/27/2025 at 11:15 PM Licensing Program Analyst (LPA), Mahjoba Mohsini, made an unannounced visit to deliver finding on the above referenced allegations. LPA met with Director/Principal Camille Stern. LPA toured the facility and observed 47 children with 16 staff in 4 classrooms. The facility is within ratio and capacity.
During the investigation, LPA conducted observation, interviewed staff and reviewed relevant documentation. Statements obtained during interviews were contradictory for both allegations and LPA did not observe any evidence that the facility wasn’t properly sanitizing. However, there remains a possibility that one or both of these allegations occurred during a single or unobserved occasion. Therefore, these allegations are Unsubstantiated. A finding of Unsubstantiated means that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.
No deficiencies are cited.
