1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 07/21/2023 at 11:40am, Licensing Program Analyst (LPA) Selina Siao conducted an annual inspection. Upon arrival, LPA met with Program Specialist, Sarah Mannos and proceeded to tour the facility. The following ratios were observed Room 6 (young infant room) had 8 infants supervised by teacher Vanessa Cuevas Lopez and teacher aide Carina Champman. Room 5 (older infant room) had 8 infants supervised by teachers Tyce Providence, Julie Kenniston and teacher aide Cindy Rivera. Director Mary Rolfes arrived to the facility during the inspection. Appropriates ratio were observed. All required notices, forms and license were posted. Furniture and age appropriate equipment is in good condition. Rooms have adequate heating, lighting, ventilation and drinking water is available for the children. Storage cubbies are readily available, and room accommodates class size. Napping equipment consists of cribs for the infants. Food service area consists of a kitchen which is clean and free of hazards. Menu is posted. Adequate food is available for meals and snacks. The infants bottles and food that are brought to the facility has a label with the child's name and date. Cleaning supplies are not accessible to children. Outdoor play area is a fenced playground with rubber top as cushioning under the play structure. Area has a canopy used for shade. Equipment is age appropriate and separated by age groups, infant and preschool. Children has their own water bottle or zippy cup when they are inside the classroom and outside at the playground. Facility uses a digital sign in and out system. First aid supplies and reviewed medication policy and storage, all areas are within compliance. Isolation area is the Director's office. A random sample of the staff and children's records were reviewed, Staff members have the required measles and pertussis immunization and has a valid online mandated reporter training certificate. The infants has an updated needs and service plan, Individual sleeping plan and sleep log available for review. Reporting requirements was also reviewed. All staff members have required criminal record and child abuse index clearances or exemptions. LPA reviewed Emergency Disaster Plan and the last fire drill was conducted on 06/21/2023. Facility has an operating carbon monoxide detector located in the kitchen above the kitchen entrance. Facility has at least one staff member that has a valid EMSA approved pediatric CPR/FA when children are in care.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.

The following items were discussed with provider: Director was reminded that corporal punishment, smoking, exersaucers, bouncy seats, walkers, and jumpers are not allowed in day care. The ABC’S of Safe Sleep: Sleep is Safest: Alone, on their Back in an empty Crib on a firm mattress.

Child Care Providers can now sign up for Quarterly Updates and PINS through the DSS website at https://cdss.ca.gov/inforesources/community-care-licensing/subscribe. LPA discussed California Megan's Law with provider and advised her to go on the website at www.meganslaw.ca.gov.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

All unusual incident reports shall be submitted to Licensing office via email at SDIncidentReports@dss.ca.gov or via fax at (619)767-2203.

The facility appears to be in substantial compliance.

Exit interview conducted and report was reviewed with the Director.

A notice of site visit was given and must remain posted for 30 days.
