25 | On February 10, 2026, at 10:45 a.m., Licensing Program Analyst (LPA) Cindy Meier conducted an unannounced Case Management visit for the purpose of follow-up on an Unusual Incident Report that was submitted to the San Diego Regional Office. LPA met with Facility Representative, Site Supervisor, Veronica Chavez and discussed the purpose of the inspection and led on a tour of the facility. There were fifty-eight (58) children present with eight (8) staff members at the time of the inspection.
On 1/15/2026 around 8:45 a.m., Parent (P1) of Child (C1) approached Staff (S1) and Staff (S2) and communicated that on 1/14/26, C1 told her that a teacher pulled her arm. It was alleged that on 1/14/26 during nap time Staff (S4) was helping C1 and pulled her arm.
Video footage of the day was viewed by Area Manager, (S3). S3 stated after viewing footage, no observable incident consistent with report was identified on the recordings. Video footage was unavailable for review by Site Supervisor or LPA. During the investigation, LPA obtained facility records, interviewed Site Supervisor, S1, S2, S3 and C1.
Based on interviews from Site Supervisor, S1, S2, S3, and C1, it was determined that although S4 may have pulled C1’s arm, no observation of this action was viewed, and it was determined that the Personal Rights of C1 were not violated.
A notice of site visit was given and must remain posted for 30 days.
Exit interview conducted and report was reviewed with the Facility Representative, Site Director, Veronica Chavez.
