| On 12/5/22 at 2:30 PM, Licensing Program Analyst (LPA) Keturah Lane made an unannounced case management inspection for reported Lead Exceedance. LPA met with Director Leonor Kennaly and toured the facility. Also present in the facility were 30 daycare children and 3 staff members in 3 classrooms. Facility was within ratio and capacity. All staff were fingerprint cleared and associated to the facility. LPA interviewed Director and examined the drinking fountain outside the women’s restroom near sanctuary deemed an Action Level/Exceedance. Water testing date was 10/15/22. The drinking fountain had lead exceedance levels of 8.7 Ug/L.
Director reported that the preschool does not use the other building where the sanctuary is located. The public uses the building on Sundays. Results from the water testing were received on 11/30/22. Director stated that the water fountain deemed an action level exceedance was bagged and labeled “Under Repair” since 11/30/22. Director stated she is unsure if the water fountain was used by the public prior to testing. Director stated she is not sure if the church facility is planning to repair the fountain or if it will remain closed. Director stated that facility manager Adrian Hom is responsible for the facilities in the other building.
None of the fountains or sinks used for drinking or food preparation in the licensed preschool area had a lead exceedance. The drinking fountain outside the women’s restroom by the sanctuary is the only outlet that had a lead exceedance.

Exit interview conducted and report was reviewed with the facility representative Director Leonor Kennaly. A notice of site visit was given and must remain posted for 30 days.
