On 7/6/26 at 1:30 pm, Licensing Program Analyst (LPA) Gerald Poindexter made an unannounced visit for the complaint received on 5/27/26 for the purpose of delivering findings on the above reference allegation. LPA met with Katrina Hoffman, assistant director. The center director, Celida Banuelos, was away on scheduled leave. The following ratios were observed today: 26 children (7 napping) with 7 staff in 3 rooms. Facility is with ratio and capacity.

During the course of the investigation, LPA Poindexter toured the facility’s exterior and interior, including areas where the anonymous Reporting Party (RP) identified in the complaint. The LPA also interviewed the center director, facility staff, and reviewed facility documents.

It was alleged by the RP that “staff did not ensure the facility was free of rodents.” As the RP was completely anonymous without contact information, LPA Poindexter was unable to interview them. Per the

complaint details submitted online, “a mouse was seen in the infant room while children present.” Based on staff interviews, it was determined that on 5/26/26, a rodent was seen by staff. The mouse crossed from the infant room into the two-year-olds classroom, while a child was arriving for care. In the weeks prior to the incident, facility staff observed the appearance of mouse droppings and chewed personal items in their break room. The center director Celida Banuelos was alerted and per protocol, placed a work order with facility maintenance. A pest control company was contacted for “urgent response” service and inspected the center on 5/9/26 and 5/18/26. The company placed what it described as “bait stations” and glue traps throughout the facility’s areas of concern. Following the most recent rodent sighting, the company was called for “urgent response” service and returned to the facility on 5/27/26. Staff interviewed were aware of the recent rodent activity and their feedback indicated that it was an uncommon occurrence. LPA also reviewed facility documentation for pest control services that were retained both before and after the rodent incident on 5/26/26. Ms. Banuelos noted that the facility carefully pursues remedies as she is wary of those pest control poisons or other methods that may be harmful to children. Based on interviews and facility documentation, it is determined that the above allegation cannot be proven or disproven, and therefore, it is considered UNSUBSTANTIATED. A finding that the allegation is unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred.

Exit interview conducted and report was reviewed with Katrina Hoffman, assistant director. A Notice of Site Visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Appeal rights were provided.
