25 | On October 29, 2024 at 12:30PM, Licensing Program Analyst (LPA) William Chancellor arrived unannounced to the facility to conduct a case management regarding an unusual incident report received on 10/15/24. The incident involved a child (C1) falling off a play structure on the playground, requiring medical attention for a fractured arm. LPA met with Director (DIR) Areli Sandoval, stated the reason for the visit and conducted a tour of the facility. Appropriate ratios and supervision were observed.
During the visit, LPA conducted interview's with two staff (S1- S2), Child 1 (C1), made observations of the playground and collected documents regarding the incident. Interview and records reviewed, revealed that staff were in ratio at the time of the incident and S2 witnessed the incident. Three of three interviews corroborated that C1 fell off the "large steps" part of the structure, falling down, onto their arm.
Facility notified the parents immediately and child obtained medical attention the next day. To improve the supervision of children when on the playground structure, staff were provided training regarding zones and one-at-a- time climbing. Based on the information gathered, LPA is unable to determine if the facility is not in violation of Title 22 Regulations at this time. The following documents are pending review by Community Care Licensing (CCL): playground structure manual confirming age restrictions and a previous Waiver approved by CCL: San Diego Regional Office.
An exit interview was conducted, a copy of this report, LIC 811 (Confidential Names List) and appeal rights were reviewed with and provided to Director Areli Sandoval. A notice of site visit was also provided and must remain posted for 30 days.
