25 | On 3/6/2025, Licensing Program Analysts (LPAs) Nancy Diaz and Hector Canton conducted an unannounced inspection. LPA disclosed the purpose of the inspection and was granted facility entry. LPAs toured the facility with lead staff, Lisa Azize. Observed present today were 17 infants in Room #4. Program operates Monday-Friday; 7AM to 6PM. Staff present today were Mitra Yazdanibiouki, Alicia Passaretti, Karina Hernandez, Valerie Klein, Yvonne Ocampo & Jackie Bulkowski.
Included in today's discussion was the unusual incident that happened on 2/24/2025 when the fire alarm went off. Five staff evacuated with 10 infants, leaving two infants who were napping in the crib area. Two staff immediately returned to the classroom and grabbed the two remaining napping infants. Staff stated that the two infants were left unsupervised for approximately 30 seconds.
A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse clearances or exemptions. The licensee has not exceeded the conditions, limitations and capacity specified on the license. Site Director, Ass't Director and several staff are trained in CPR and Pediatric First aid, copies of certificates were observed on file.
Disinfectants, cleaning solutions, poisons and other items that are dangerous to children are inaccessible. Medications are kept in a safe place, inaccessible to children. Furniture and equipment are in good condition, free of sharp, loose or pointed parts. All toilets, handwashing facilities are in safe and sanitary operating condition. All floors are clean and safe. The child care center was observed to be clean, safe, sanitary and in good repair to ensure the safety and well-being of children, employees and visitors. Facility maintains a carbon monoxide detector that meet the standards established in Chapter 8 of Part 2 of Division 12.
